NEW REGISTRATION/ENQUIRES

The practice has an open list. We ask that all enquires for new registrations are made between 10-11am and 4.30-5.30pm

The following documents are required to register with the practice:

  • Your medical card
  • Photographic identification (passport or driving licence)
  • One proof of address (utility bill/ bank statement)
  • One proof of residency (benefits/child/housing/Council tax/contract of housing, current payslips/proof of payment for student fees)
